Getting Started with TEAM
Two pages of information are included here for the convenience
of parents who might be interested in having their child
attend sessions at TEAM Learning Center. Parents are urged to
call the center prior to using these forms so that we can
begin the process of becoming acquainted with you. The
registration process normally involves an in-depth
introductory phone interview and a review of pertinent
personal information, previous testing and report cards, and
examples of current school work. Once the initial interview
phase has been completed these forms may be completed and then
e-mailed or faxed to TEAM.
